Transitioning Your Practice to Private Pay

For many private practitioners, the dream of a fully private pay practice is very appealing—freedom from low reimbursement rates, time-consuming paperwork, and the red tape of insurance claims. But shifting to private pay isn’t something you do overnight. It’s a process that requires thoughtful planning, strategic decision-making, and clear communication with your clients.

If you’ve been thinking about making the leap, know that there is a path forward that doesn’t involve financial panic or patient mass exodus. 

Start Slow

Before going fully private pay, many successful practitioners begin with a hybrid model. This model allows you to continue accepting some insurance while also building your private pay caseload. It's a safer, more manageable way to:

  • Test the waters with your marketing and messaging
  • Adjust your pricing and practice policies
  • Build confidence in communicating the value of your services outside of insurance

In short, a hybrid model allows you to gather data, observe trends, and prepare your systems and clients for the eventual transition.

How to Choose Which Carriers to Drop

Not all insurance contracts are created equal. If you are planning to gradually reduce your insurance load, it is essential to first evaluate your current contracts.

  • Reimbursement Rate - Look at the rate you're being paid per session across your insurance contracts
  • Number of Active Clients - Look at the number of current clients you have with each carrier (dropping a carrier with only 2-3 clients requires a different strategy that one that represents 30% of your caseload)
  • Administrative Burden - Which carriers require a disproportionate amount of time following up on claims, battling denials, etc.?
  • Timeliness of Payment - Delays in pay directly affect your cash flow. Which carriers are chronic late-payers or continually request records?

Moving to private pay is a long game, not a quick fix. It’s not about abandoning your clients or taking a financial leap of faith—it’s about aligning your business model with your values, your worth, and your long-term goals.
