The Shift Toward Private Pay

As a private practice coach, one of the most pressing conversations I’m having with therapy practice owners lately is this: We can no longer count on third-party insurance to sustain a thriving business.

Whether you're a speech-language pathologist, occupational therapist, mental health counselor, or physical therapist, the trend is clear—insurance reimbursements are declining in value, increasing in administrative burden, and placing unnecessary restrictions on the care we provide. For many, it has become nearly impossible to grow (let alone sustain) a private practice without making a strategic pivot toward private pay and revenue diversification.

Why Insurance is No Longer a Reliable Foundation

Over the last few years, therapists have watched insurance companies:

  • Reduce reimbursement rates, sometimes below the cost of providing care.

  • Increase denials and audit risks, creating administrative burdens that require non-billable staff time.

  • Limit the number and type of sessions clients can receive, regardless of clinical need.

  • Delay payments, complicating cash flow and budgeting for the practice.

These issues have made it clear: depending entirely on insurance contracts leaves your practice vulnerable. As cost of living, staffing, rent, and software fees rise, insurance simply isn't keeping up.

Embracing a Private Pay Model

Shifting to a private pay model isn’t just about charging clients directly—it’s about reclaiming your autonomy as a clinician and business owner. In a private pay practice:

  • You set your rates based on the true value of your work.

  • You decide how long sessions are, how often to see clients, and what approaches you use.

  • You serve Right-Fit Clients who are invested in the care they’re receiving.

  • You gain more predictability and stability in your revenue.

It’s not an overnight shift—but with transparency, education, and value-driven services, clients will understand and often prefer the direct-pay model.
